This Privacy Policy explains how Axio Anesthesia collects, uses, and protects information through this website (axioanesthesia.com) and our text messaging (SMS) program. Protected health information we receive as part of your medical care is also governed by our separate Notice of Privacy Practices under HIPAA, available on request by emailing info@axioanesthesia.com. If this policy and the Notice of Privacy Practices conflict regarding health information, the Notice of Privacy Practices controls.
1. Information we collect
- Information you give us: name, mobile phone number, email address, and any message you send through our forms or by text.
- SMS program information: your mobile number, the date/time and method of your opt-in, and opt-out and HELP requests.
- Technical information: IP address, browser type, and pages visited, collected through standard server logs.
2. How we use information
- To respond to your requests and schedule or confirm appointments and callbacks.
- To send the text messages you agreed to receive (appointment and procedure reminders, callback and scheduling messages, office notifications).
- To operate, secure, and improve our website and services, and to comply with law.
3. Text messaging (SMS)
- If you opt in, Axio Anesthesia will send text messages about appointments, callbacks, and office notifications. Message frequency varies. Message and data rates may apply.
- Reply STOP to opt out at any time; reply HELP for help, or contact us at info@axioanesthesia.com.
- Consent to receive text messages is not a condition of receiving care.
- We do not sell, rent, or share your mobile phone number, SMS opt-in data, or messaging consent with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties, except service providers (such as our messaging platform) that process it solely to deliver our messages to you. All other categories of sharing in this policy exclude text messaging opt-in data and consent.
4. How we share information
We share information only with service providers who help us operate our website, phone, and messaging services and who are required to protect it; when required by law; or to protect rights and safety. We do not sell personal information.
5. Please do not send sensitive medical information
Standard text messages and website forms are not a secure way to send detailed health information. Please do not include diagnoses, medications, or other sensitive medical details in texts or website forms; call our office instead. Our texts will contain only limited information needed for scheduling and office communication.
6. Data security and retention
We use reasonable administrative, technical, and physical safeguards. We keep information only as long as needed for the purposes above or as required by law.
7. Your choices
You may opt out of texts at any time by replying STOP. You may ask to access or correct the information you provided by contacting us.
8. Children
This website is not directed to children under 13. Parents or guardians may contact us on behalf of a minor patient.
